Main responsibilities:
Compliance with specific individual customer care plans and reporting on the completion of tasks.
Assistance in meeting the personal needs of clients (bathing, dressing, etc.).
Full-fledged performance or assistance in regular home cleaning, cooking and laundry.
Transportation of the client to the doctor, to the grocery store and on other important errands.
Assistance to the client in self-medication.
Monitoring and reporting of all changes in the client’s status.
Constant observance of strict confidentiality in connection with access to personal data.
Implementation of regular, pre-agreed, permanent care and adaptability to meet the needs of customers.
Compliance with all rules and requirements of the Medicare, Medicaid and HIPAA programs.
Compliance with all rules, policies, procedures and standards.
Execution of other assignments.
Requirements for the position and competence:
Age over 18 years.
The presence of a certificate of a home care specialist or a personal care specialist obtained as a result of taking courses in the home health care program approved by the New York State Department of Health, with confirmation of an entry in the register of home care services of the State of New York.
Medical examination before employment / PPD test before contact with the patient.
Passing a criminal record check in accordance with the rules of the New York State Department of Health.
Availability of reliable transport (a valid driver’s license and insurance are required for the car).
